Six days ago, an FDA advisory committee voted 8 to 5, with one abstention, to recommend Semax for the 503A compounding list. A Russian stroke drug most Americans know from nootropics forums just got a favorable federal-panel vote, and it landed the same week we finished repricing Semax across every vendor we track: 47 sellers, from $1.17 to $8.50 per milligram. If you want the short version of where we would spend, Atomik Labz and NuLife Peptides are the two we keep coming back to, and the reasons are below.

The vote that just happened

On July 24, 2026, the FDA's Pharmacy Compounding Advisory Committee reviewed Semax for three uses: cerebral ischemia, migraine, and trigeminal neuralgia. The committee recommended it for the 503A Bulks List, which would let compounding pharmacies work with it legally. Semax was one of six peptides that cleared that day, alongside BPC-157, TB-500, KPV, MOTS-c, and Epitalon.

Two things are worth keeping straight. FDA's own scientific reviewers had recommended against listing all seven compounds on the agenda, citing thin clinical data, and the committee went its own way on six of them. And an advisory vote is a recommendation, so the legal status of Semax in the US today is exactly what it was last month: sold for research purposes only, with no FDA approval. Formal listing would take a separate rulemaking with public comment. A second peptide hearing is expected in February 2027. A $73 spread on the same 10mg vial

Here is what the pricing looks like once you strip out the marketing. Across the 47 vendors carrying Semax in our index, price per milligram runs from $1.17 at the floor to $8.50 at the ceiling, with a median of $3.60. The 10mg vial is the workhorse size, stocked by 30 of those 47 sellers, so in practical terms you are choosing between a roughly $12 vial and a roughly $85 vial of the same seven amino acids.

That spread is the entire argument for shopping on price per milligram instead of sticker price. A 30mg vial at $60 beats a 10mg vial at $35, and vendors know most people do that math in their heads and get it wrong. Check the label before you compare prices

Six of the 47 vendors carry N-Acetyl Semax Amidate, a modified version of the same peptide, and a handful list it alongside plain Semax under one product name. The modified version caps the peptide at both ends, an acetyl group at the N-terminus and an amide at the C-terminus, which slows enzymatic breakdown and is generally sold as the longer-acting option. Our take: the acetylated version is a legitimate product with a real chemical rationale, and its price per milligram belongs in a separate column from plain Semax. Comparing the two directly makes a cheap vial look cheaper than it is.

What the research actually shows

Semax comes out of the Institute of Molecular Genetics in Moscow, built from the ACTH(4-10) fragment with a Pro-Gly-Pro tail bolted on for stability. Russia has had it registered as a drug since the 1990s, mostly as a nasal spray for stroke recovery and cognitive complaints, which is where nearly all the human data comes from.

The mechanism work is more convincing than most peptides in this category. In a 2006 study in Brain Research, a single 50 mcg/kg intranasal dose in rats raised hippocampal BDNF protein about 1.4 fold and trkB activation 1.6 fold, tripled exon III BDNF messenger RNA, and produced measurably better performance on a conditioned avoidance task. A companion paper in the Journal of Neurochemistry the same year found Semax binds specifically in the rat basal forebrain and raises BDNF protein there.

On the clinical side, a 2018 paper in Zhurnal Nevrologii i Psikhiatrii followed 110 ischemic stroke patients, mean age 58, on two 10 day courses of 6,000 mcg per day with a 20 day gap. Plasma BDNF rose in the treated groups at both early and late rehabilitation stages, and higher BDNF tracked with better Barthel index scores. The honest framing: this is a Russian-language literature with limited Western replication, and the animal mechanism data is stronger than the human outcome data. We would rather say that plainly than dress it up.
